Get a pre-signed upload URL POST
Create a pending media upload intent and generate a create-only pre-signed R2 PUT URL. Send every header in upload_headers exactly as returned, including Content-Type and If-None-Match, then call POST /v1/media/confirm; confirmation is mandatory before using the canonical media URL.
Upload a file POST
Upload a raw file body. Pass the filename as a query parameter and set Content-Type to one of the documented media types. Generic application/octet-stream bodies are rejected.
